The Blues took the lead through a Jimmy Floyd Hasselbaink penalty before Jermain Defore equalised before half-time.
West Ham took the lead on 48 minutes through Paolo Di Canio however Chelsea looked to have secured a draw as fellow Italian Gianfranco Zola scored in the 74th minute.
However the drama was not over as West Ham legend Di Canio scored his second of the game with six minutes to go.
